ICM- Integrated Case Management Service

A community matron can visit you in your own home in order to reduce your risk of going into hospital. They will then discuss your case in a special meeting with the GP to try and improve the care that you receive. The GP can refer you for this service or you can also refer yourself. Community Treatment Team

The community treatment team works with adults in the community with an acute medical need who could potentially be treated at home, rather than attend accident and emergency (A&E).

The aim of the Community treatement team (CTT) service is to prevent unnecessary hospital admissions.

The service runs 8am- 10pm 7 days a week. Patients can self refer

Help not Hospital

The British Red Cross is working with Havering Borough to provide help to the elderly in periods of crisis. They can provide time limited support and help with things like shopping, paying bills, preparing light meals and picking up prescriptions.
